Job Summary:

The Institute of Continues Learning in the Department of Nursing at SUNY Downstate Health Sciences University is seeking a full-time TH Assistant Director of Nursing / Wound and Ostomy Care Nurse. The successful candidate will:
- Develop and manage an individualized wound care treatment plan for optimal healing outcomes or chronic maintenance
- Demonstrate knowledge of up-to-date topical wound care products and therapies, including ordering, application, and management of the products on wounds and ostomies, also including negative pressure wound therapy
- Provide hands-on wound care to patients, including debridement of wounds by enzymatic and autolytic methods when appropriate
- Order, review, interpret; and integrate diagnostic studies and laboratory findings for initiating or modifying the treatment plan
- Collaborate with the patient primary care provider when comorbidities are contributing to or affecting the healing of a wound to determine a comprehensive plan
- Update the primary care provider on the status and management plan of the wound as appropriate
- Prescribes medications and/or durable medical equipment to assist in healing wounds
- Assist with the identification of wound care quality measures and provide data and documentation for tracking quality measures
- Work with nursing leadership and the Director of Nursing Education to determine opportunities for education of nursing staff with appropriate identification of wound types, prevention of wounds, and nursing care of wounds
- Develop wound care education plans in collaboration with nursing leadership
- Instruct patient/family regarding wound treatments and management plan for optimal wound outcomes
- Triage telephone calls and provides consultation and serves as a resource to the frontline nursing staff
- Participate in professional development activities and maintains professional affiliation
