Job Summary:

The Department of Transplant Surgery at SUNY Downstate Health Sciences University is seeking a full-time TH Assistant Director of Nursing / Nurse Practitioner. The successful candidate will:

• Attend post-transplant clinic and perform independent clinical assessments of transplant recipients.
• Review laboratory, radiographic and diagnostic test results; monitor for adverse effects and changes which could affect the patient.
• Collaborate with attending surgeons and neurologist related to results and plan of care.
• Maintain continuity of care between inpatient and outpatient services.
• Provide patient education related to post-transplant care.
• Develop and implement protocols for health maintenance, screening and quality of life management after organ transplantation.
• Create referrals to subspecialty care as needed.
• Participate in multidisciplinary care coordination.
• Participate and assist with quality improvement initiative and programs.  
• Perform additional duties/responsibilities as required and/or needed by the department. 

Required Qualifications:

• New York Registered Nurse and Adult/Family Nurse Practitioner Licensures.
• Master's Degree in Nursing.
• 10+ years of nursing experience.
• Current national certification as an Adult/Family Nurse Practitioner.

Preferred Qualifications:

• 3+ years of experience as a Nurse Practitioner in an outpatient setting.
• Prior experience in the care of chronically ill patients.

Work Schedule:

Variable Days; 6:00am to 7:00pm
