Job Summary:

The Department of Hospital Administration at SUNY Downstate Health Sciences University is seeking a full-time Director of Care Coordination / TH Associate Administrator. The successful candidate will:

  • Oversee daily program operations with the Care Management Team (Social Work and Case Management), with a focus on hiring and supervising key program leadership and staff
  • Evaluate program and staff performances by analyzing an interpreting data and metrics
  • Direct the standardization of CM policies, practices workflow and data reporting within the system
  • Establish strategic. tactical and financial plans to optimize the efficacy of clinical case management operations
  • Remove barriers lo optimal performance, including: decreasing length of stay, reducing readmissions, ensuring regulatory compliance, reducing denials, enhancing population health management
  • Lead the implementation of improved clinical care management models and utilization management systems to: decrease readmissions, recidivism, and excessive length of stay and medical denials; respond to regulatory changes
  • Collaborate with other revenue cycle leaders to ensure processes are aligned
  • Provide accurate and pertinent information to appropriate departments, administration, medical staff, and others involved with CM activities
  • Collaboration of ED utilization/direct admits process coordination; understands compliance from a Case Management perspective
  • Evaluate the facility/practice case management care coordination operations as well as individual core processes to improve the performance and financial expectations
  • Review data for trends and analyzes the impact on department operations
  • Develop and implement key training tools for key leadership and staff positions
  • Perform other job-related duties as assigned/needed

Required Qualifications:

  • Clinical experienced Registered Nurse, Case Manager or Licensed Social Worker
  • Master's Degree with a strong concentration in healthcare, strategic and financial planning
  • Experience with Brooklyn's healthcare landscape.
  • Knowledge of federal and state healthcare initiatives and regulations, including JCR/CMS
  • Proficiency with detailed project plans, budgets and financial reporting.

Preferred Qualifications:

  • 8+ years of project or program management experience with certification preferred

Work Schedule:

Monday to Friday; 8:30am to 4:30pm (Full-Time)

MP-3

Salary Range:

Commensurate with experience and qualifications
